DO NOT fill out more than one form. The google form includes the BCS Foods Permission and Release form, the JROTC Health and Responsibility Agreement, the JROTC Memorandum of Agreement, the JROTC Photo Waiver, the JROTC Health Survey, and the JROTC Cadet Detail Form. Cadets cannot participate in JROTC Activities until the form is completed.
